One-hour Healthy Eating seminar.

• For seminar,1 Health Educator, preparation, teaching tools/visuals, handouts and presentation.
• Summary of seminar participant results will be reported, with a minimum of 10 evaluations.
• Maximum of 50 participants.
• Round trip travel from Kaiser Permanente Santa Clara Medical Center to Town of Los Altos Hills campus and return.
Customer will provide the following equipment needs:
• Space for seminar presentation/screening
• Tables and chairs for seminar/screening
• Table for educational materials
• Access to 3 -prong electricity for computer/projector/screening equipment, within 7 feet.
• Laptop/computer
• Projector
• Screen/white space or wall for projected slides
• Chart pad/dry erase board
Cancellation and Reschedule Policy
Cancellation or reschedule request must be received at least 2 weeks before the event in order to incur no charge. Charges are as follows:
■ Reschedule within 10 business days: 25% estimated fees
■ Cancellation within 10 business days: 50% estimated fees
■ Cancellation within 5 business days: 100% estimated fees
Payment Terms
Payment due no later than 30 business days from date of invoice. Make checks payable to: The Permanente Medical Group, Inc. and
