CITY SEAL/LOGO RELEASE FORM
The undersigned as legal representative of Los altos Hills (dq/town) hereby grants to
Remmel Consulting (RC) and Orange Circle Consulting (OCC) the right to use the logo and/or
city seal for photo and video representation in the following digital work:
Santa Clara County Emergency Volunteer Center E -learning Training Series
The undersigned waives the right to inspect or approve any such use of his/her city logo/seal,
releases RC and OCC and all assignees, licenses, or successors of OCC from any claim or liability
arising out of such usage.
