ANY AND ALL SERVICES PROVIDED BELOW ARE DEFINED IN AND SUBJECT TO THE MASTER TERMS AND CONDITIONS
This Rider is intended to be made a part of that certain Schedule of Protection, Proposal and Sales Agreement ("Agreement")
between Protection One, a division of ADT LLC, ("Protection One") and the Customer listed above for service at the location(s)
listed below.
The Customer hereby agrees to pay Protection One, its Agents or Assigns, the sum of $0.00 plus any applicable taxes, of which
$0.00 shall be payable upon signing of this Rider and the balance payable upon completion of installation. In addition, Customer
agrees to pay Monthly in advance the additional sum of $20.00. Customer agrees to pay the total Licenses and Permit Charge of
$0.00 at the time of sale.
The parties agree that the Agreement of which this Rider is made a part is and shall remain in full force and effect in accordance
with all the terms and conditions thereof, modified only as specifically provided in this Rider.
To the extent that there is any conflict or inconsistency between the master terms and conditions and this rider, the terms and
conditions of this rider shall control.

services to you. You acknowledge that you may not receive a copy of this Contract signed by Protection One's Authorized Manager, but such lack of receipt shall not, in anyway,
invalidate or otherwise affect this Contract.
