You are hereby directed to make the herein described changes from the plans and specifications or do the following described work not
included in the plans and specifications on your contract.
DESCRIPTION OF WORK
This final balancing change order is issued to pay the additional AC, pay the changes of traffic stripping on the bridge, settle potential
claim, and adjust the contract bid quantities to as -built quantities. The parties herein agree that this final contract adjustment resolve all
claims and issues.
1. The final quantity of Bid Item # 11, Asphalt Concrete is 45.51 tons. The additional quantity of 20.5 tons over 125% of the
original quantity will be paid with agreed unit price of $200/Ton. The additional payment for the AC is therefore $4,100.00
2. Credit to the Town for deleting 6" concrete curbs on the bridge
3. Install traffic striping and buttons on the bridge
4. Settle the potential claim by HSD and the Town agreed to pay
5. Bid Quantity Adjustment (per attached schedule summary)
We, the undersigned contractor, have given careful consideration to the change
proposed and hereby agree, if this proposal is approved, that we will provide all
equipment, furnish all materials, except as may otherwise be noted above, and
perform all services necessary for the work above specified, and will accept as full
payment therefore the prices shown above.
