You are hereby directed to make the herein described changes from the plans and specifications or do the following described work not
included in the plans and specifications on your contract.
DESCRIPTION OF WORK
The following is a description of work to be done and price to be paid.
Partial removal of existing concrete abutments at western half of the bridge which was not required on the project plan
Jones Bros. has furnished labor, equipment and material on October 2 and 3, 2002 to perform above -referenced work. The cost
information was based on approved Daily Extra Worlc Report No. 5 and No. 6. This work is to be done at the Lump Sum of:
$3,226.95
We, the undersigned contractor, have given careful consideration to the change
proposed and hereby agree, if this proposal is approved, that we will provide all
equipment, furnish all materials, except as may otherwise be noted above, and
perform all services necessary for the work above specified, and will accept as full
payment therefore the prices shown above.
By reason of this proposed change, o day4 extension of time will be allowed.
